After a little more research, I got a preliminary H.264 patch that fixes
the blockiness and time delay ... I still have a problem with the frame
rate, which is way too low, but I think it's sufficient to make the
codec usable.  I've bundled up the patch and added it to
https://bugzilla.gnome.org/show_bug.cgi?id=596006 so we'll see if they
take it.

Next step will be to analyse exactly what Gnome:Factory has done and see
if I can make the package split work.
